Thanks for your excellent summary, Dr. John! I have learned so much from you and the other doctors on this list. I just have one comment:

While the above may not cause reflux, if your esophagus is already sore from reflux, pouring acid on the wound, so to speak, can be exceedingly painful! For those of you who don't have reflux, imagine having a cut on your finger and then peeling an orange or a lemon and getting some of the juice in the cut.
