Yes! It's a huge problem for me. I used to be able to sight read
fairly well while I was singing regularly in choirs, but a good ten
years away from choir singing has destroyed that ability. Now I'm
focusing so much on getting the notes and rhythm right that my
technique disappears completely.

The only thing that I have found that helps is if I:

a) familiarise myself with the text and rhythm before singing the piece
b) listen to the piece before I sing it

I can barely hack out the melody line with my right hand on the piano,
so accompanying myself is not an option. I do find, however, that I
learn the piece much better if I invest the time and learn it with the
piano. The CD then becomes a reference point which I listen to
occassionally.
