Most nursing homes have an activities director. Call and ask for that person and discuss your wishes with her/him. It's unlikely that the home could provide an accompanist for you, although there are many retired musicians living in retirement complexes, so you might check with one of those and see if there's someone who would enjoy playing for you on your nursing home visits. It would be ideal if the retirement unit and the nursing home unit were part of one complex, and that is often the case.
